UFC is offering a free Tapout t-shirt with purchase of four tickets to the UFC 118 simulcast being presented by Fathom Events this Saturday. To receive the tshirt, you would need to fill out the following with all four ticket stubs attached:

Instructions:
Purchase four (4) tickets to attend the UFC® Pay-Per-View event at a participating movie theater near you scheduled to be held on 8/28/10. Then, send in your four (4) ticket stubs to the address noted in the redemption form and receive a TapouT T-shirt.
To redeem your TapouT T-shirt:
1. Purchase four (4) tickets from an authorized ticket seller to attend the UFC® 118 Pay-Per-View event at a participating movie theater near you scheduled to be held on 8/28/10.
3.Complete the redemption form.
4. Make a photocopy of your ticket stubs and completed redemption form to keep for your records.
5.Mail all original items together in an outer mailing envelope with appropriate postage affixed to be postmarked by 10/20/10 and received by 10/27/10 to:
